How many patients were removed from GP practice lists in each year following list reconciliation, validation or cleansing processes (including removals following FP69 flags, Personal Demographics Service tracing, or equivalent processes)? Please break down removals by reason where recorded — for example: deceased, moved or untraced at registered address, duplicate registration, embarkation/left the country. 2. What was the average annual payment made to GP practices per registered patient in each year (global sum per weighted patient and, if readily available, average total funding per registered patient)? 3. What is the most recent estimate held by NHS England of the number of patients registered with GP practices who are deceased, no longer resident at their registered address, or no longer resident in England, and of the annual cost of capitation payments made in respect of such patients? 4. How much funding was recovered from, or deducted from future payments to, GP practices in each year as a result of patients being removed from lists retrospectively?
