Request   I am making this request under the Freedom of Information Act 2000.   This request relates to the Northern Ireland Neurology Recall from 2018 to present, including any related information held, received, shared or handled by your organisation in connection with NHS patient data, NHS numbers, demographic records, GP registration data, NHS Spine/PDS records, prescribing data, prescription services, audit trails,coding, cross-border NHS data flows, complaints, litigation handling, record disclosure or related information governance.   Please answer the following questions YES or NO only.   Has your organisation ever held, received, shared, accessed, processed, or been involved in handling any information relating to the Northern Ireland Neurology Recall or its aftermath?   YES / NO   Has your organisation ever held, received, shared, accessed, processed, or been involved in handling any information relating to Northern Ireland Neurology Recall patients in connection with NHS numbers, demographic records, GP registration data, NHS Spine/PDS records, GP Summary/Summary Care Record information, prescribing data, audit trails or cross-border NHS data flows?   YES / NO   Has any Neurology Recall-related information ever been treated by your organisation as restricted, sensitive, national security sensitive, intelligence sensitive, security-body related, or subject to special handling instructions?   YES / NO   Has your organisation ever relied upon, considered relying upon, or been advised to rely upon any FOIA exemption in relation to Neurology Recall-related information, including section 23, section 24, section 30 or section 31?   YES / NO   Has your organisation ever applied a “neither confirm nor deny” position in relation to Neurology Recall-related information?   YES / NO   Has any Neurology Recall-related information ever been handled under Closed Material Procedures, Public Interest Immunity, restricted disclosure arrangements, national security handling, intelligence-sensitive handling, cross-government restricted handling, or inter-agency restricted handling instructions?   YES / NO   Has any other public authority, department, NHS body, security body, legal representative, insurer, indemnifier, or external body influenced, directed, advised or requested how your organisation should handle, classify, restrict, disclose or exempt Neurology Recall-related information?   YES / NO   Has your organisation ever been asked by any Northern Ireland public authority, UK Government department, NHS body, police body, legal representative, insurer, indemnifier, or regulator to preserve, restrict, amend, suppress, review, withhold, redact, classify, or specially handle information relating to Neurology Recall patients or their NHS records?   YES / NO   If the answer to any question is YES, please state which category applies.   If any part of this request is refused, please specify the FOIA exemption relied upon and whether the refusal relates to the information itself or to confirming or denying whether information is held.   For clarity, I am not requesting personal medical information about any patient under this FOI request.
