#Request _Please treat this as a request under the Freedom of Information Act 2000._ _For each of the last five financial years (and the current year to date), please provide:_ _1. The number of NHS BSA staff investigated for unauthorised access to personal data (e.g. prescriptions, pensions, benefits, or staff records)._ _2. The number of those cases that were substantiated._ _3. The number of staff disciplined, with a breakdown of outcomes (e.g. warning, dismissal)._ _4. The number of cases referred to the ICO or police._ _Please supply the data in CSV or XLSX format if possible._ _If providing all years would exceed the cost limit, please prioritise the last three financial years._ #Response I am writing to advise you that following a search of our paper and electronic records, I have established that the information you requested is not held by the NHSBSA. We have reviewed our employee relations records dating back to April 2019, and we have had no instances of this allegation, within this time frame.
