##Request Dental penalty surcharge 2015: What's the complete fee imposed for this at this part of time? NHS dental services but the enquiry was generic freedom of information not data protection. The penalty charge itself is 5 times the treatment charge up to £100 maximum, and the surcharge is 50% of the penalty charge. The same was true in 2015.
