#Request# could you please provide me with information relating to version control of the VDPS claim form below: I would like to know all changes made to this form and the instructions with it from November 2021 onwards. Vaccine Damage Payment Scheme application form - U16 and UMOA (V2.6) 11.2023 #Response# A copy of the information is attached. The NHS Business Services Authority (NHSBSA) took over the administration of the Vaccine Damage Payment Scheme (VDPS) from the Department for Work and Pensions on 1 November 2021. The claim form for the VDPS was transferred to us from DWP. We updated this claim form with NHSBSA branding. In March 2023 we separated the claimant journey by creating three claim forms: 1. Vaccine Damage Payment Scheme - self-claimant 2. Vaccine Damage Payment Scheme - on behalf of someone who has died 3. Vaccine Damage Payment Scheme - on behalf of someone under 16 (U16) or unable to manage their own affairs (UMOA) Three versions of the ‘Vaccine Damage Payment Scheme – U16 and UMOA’ claim form have been made available on our website: 1. 'Vaccine Damage Payment Scheme - U16 and UMOA (V1) 03.2023'. 2. 'Vaccine Damage Payment Scheme - U16 and UMOA (V2) 05.2023' was created because of a change to the VDPS's email address. 3. 'Vaccine Damage Payment Scheme - U16 and UMOA (V2.6) 11.2023' was created because additional information had to be added to the form. This included more information about power of attorney, instructional information to support with completion of the form, information about how we communicate with claimants, and additional information about consent. The claim forms are named based on the following: [file name] [version number] [month and year created].
